The Care Act 2014 imposes new duties on councils to facilitate service users moving between councils and continuing to receive the care and support they need.  From April 2016 councils will also have to transfer information about Care Accounts when self funders move.  ADASS Information Management Group consultant Richard Pantlin has worked with Leeds City Council and Calderdale Council to assess the feasibility of transferring care information electronically between councils.  The findings are contained in this report.  The content is relevant to managers with responsibility for continuity of care processes, anyone procuring systems and IT system designers.
