The national Transforming Care Programme aims to reduce reliance on inpatient beds and improve the quality of care and the quality of life for people with learning disabilities and/or autism who have behaviour that challenges, and may have mental health needs. Since the programme launched its national plan (Building the Right Support) and service model, local authorities have joined with Clinical Commissioning Groups and NHS specialised commissioners to form Transforming Care Partnerships. This workshop is an opportunity for you to discuss successes and challenges to date in your Transforming Care Partnership, with informal contributions from colleagues in Fast Track areas.
