Earlier this year, Think Local Act Personal published a new Partnership Agreement setting the framework for the next stage of personalisation across health and social care. This session will introduce the new TLAP partnership agreement and work programme and provide detail on several key priorities:

  • Personalisation and the Care Act – work underway to develop materials to support implementation
  • Building community capacity – launch of a new framework to support Health and Wellbeing Boards with PHE and the LGA
  • The Integrated Personal Commissioning Programme – in partnership with NHS England
