Title of Session: DTOC: 'Not our Fault', or 'Everyone's Business'?
A Struggling hospital... a health and social care 'system' characterised by organisational conflict... challenging budgets all round... and that pesky focus on Delayed Transfers of Care.
Rather than giving in to that recipe for blame, councils and CCGs in Barking & Dagenham, Havering and Redbridge teamed up with hospital trusts to integrate hospital discharge services. The Joint Assessment & Discharge (JAD) Team will have areal decision-making powers, across professional and organisational boundaries. Faster decisions and shared responsibility are fostering a more creative approach to difficult cases, and more personalised care planning.
The session will be a short, sharp perspective on how the team came into being and its first months of operation, demonstrating Better Care Fund principles in action: integrated care and 7-day working underpinned by a shared information system, Health Analytics, which manages workflow across the systems.
