Title:  Transforming transition - using the Children and Families Act and the Care Act to improve outcomes for young people and make better us of resources.  Learning from teh SEND pathfinders

This session will focus on learning from the SEND pathfinders (in particular Manchester and Hertfordshire) about what local authorities, partner agencies and families need to do in the transition phase to achieve better outcomes for young people with special educational needs/disabilities.

We will consider how to: 

build communities to help young people have friends and be active participants;

develop pathways into employment;

increase young people’s independence and reduce family dependency on services;

improve young people’s health outcomes.

We will look at how Education, Health and Care plans can help individuals achieve their outcomes and how local authorities and CCGs can use the information they contain to shape local provision. 

We will also discuss implications of the legislation on how services work with families and for workforce development.
